NOTICE OF ADOPTION
You are hereby notified that there has been a PETITION FOR ADOPTION filed with the Superior Court of Clayton County, Georgia, Case No. 2011-AA-01243-6 for the purpose of adoption of the minor male Child born September 24, 2010 in Fulton County, Georgia.
You are further notified that the biological mother executed and delivered a Surrender of Rights Final Release for Adoption pursuant to O.C.G.A. § 19-8-5.
In accordance with Official Code of Georgia Annotated § 19-8-12, you are hereby advised that you will lose all rights to said Child and will neither receive notice nor be entitled to object to the adoption of the Child unless within thirty (30) days of the publication of this notice you:
(1) File a Petition to Legitimate the Child pursuant to O.C.G.A. § 19-7-22 or an Acknowledgment of Legitimation pursuant to O.C.G.A § 19-7-21.1;
(2) File notice of the filing of such Petition to Legitimate or Acknowledgment of Legitimation with the Superior Court of Clayton County, State of Georgia; and
(3) Provide notice of the filing of said Petition to Legitimate or Acknowledgment of Legitimation to J. Nathan Blau, Attorney for Petitioner, 4145 Clairmont Road, Atlanta, Georgia 30341

The State of Tennessee, Department of Children's Services has filed a Petition to Terminate Parental Rights against you seeking to forever terminate your parental rights to the children, Keishla Gonzalez Vazquez, Kaitlyn Gonzalez Vazquez, and Karoline Marie Gonzalez. It appears from the record in this matter that ordinary process of law cannot be served on you because your whereabouts are unknown.
You are hereby ORDERED to appear before the Hamilton County Juvenile Court, 1600 E. 3rd Street, Chattanooga, TN 37404 for trial on March 16, 2012 @ 9 am to plead and/or answer the Petition.
On the above date, the Court will hold a hearing for the relief demanded in the Petition. If you fail to appear at the hearing, evidence will be presented in your absence, and your parental rights to the subject child could be forever terminated at the hearing. (this is for a parent represented by counsel)
Any appeal of this Court's final disposition of the complaint or petition for termination of parental rights will be governed by the provisions of Rule 8A, TRAP, which imposes special time limitations for the filing of the transcript or statement of evidence, the completion and transmission of the record on appeal, and the filing of briefs in the appellate court, as well as other special provisions for expediting the appeal.
